"SECURITY LIGHT" AGREEMENT WITH COOPERATIVE

This is to certify that I agree to pay the Cherokee County Electric Cooperative Association $8.19 per month plus Power Cost Recovery Factor and State Sales Tax, if applicable, for use of a "SECURITY LIGHT" under the following conditions:

1. The Cooperative agrees to furnish service for $8.19 per month rental fee, plus Power Cost Recovery Factor and State Sales Tax, if applicable.
2. I understand that the "SECURITY LIGHT" bill becomes delinquent if unpaid sixteen (16) days after the billing date and that "SECURITY LIGHT" Service shall be discontinued if the bill has not been paid within twenty-six (26) days from the billing date providing proper notice has been given at least ten (10) days prior to the stated date of disconnection.
3. I agree the Cherokee County Electric Cooperative Association is the sole owner of this light and will maintain it in good operating condition, which includes replacing bulbs--only during regular working hours.
4. It is agreed there are no verbal agreements regarding the installation of this light.  I have signed this contract signifying my desire to use this service.
5. Any additional poles are equipment are not included.
